runtime error terminate the application

ASSERT failure in QList<T>::operator[]: "index out of range", file ..\..\Desktop\Qt\4.8.1\mingw\include/QtCore/qlist.h, line 477
Invalid parameter passed to C runtime function.

this is the error i get when i run my sniffer , i did a qtreewidget so i put the output in it but i get this error and i dont know y

index out of range

it say everything, your app is trying to read element which not exist, e.g . list have 5 elements so index are 0,1,2,3,4, when you try to read 5 and more you get this error
