Good afternoon,
I want to make a simple application where I have a form to enter the date and time that I want my application to tell me something.
Any suggestions?

look at some qt examples and tutorials
If you have a problem, CUT and PASTE your code. Do not retype or simplify it. Give a COMPLETE and COMPILABLE example of your problem. Otherwise we are all guessing the problem from a fabrication where relevant details are often missing.

Of course it's always more fun doing it your self, you can take a look at a program I wrote some time ago that might be almost exactly what you want.
